Shin Splints: Stopping You in Your Tracks

Shin splints, or pain in the shin area, is a common condition affecting runners and those engaged in high-impact activities. The most frequent cause is Medial Tibial Stress Syndrome (MTSS), an irritation of the periosteum—the lining of the bone. MTSS is considered a bony stress reaction, which, if unchecked, can lead to a stress fracture. This condition may also involve tendinopathy or dysfunction in muscles like the tibialis posterior, tibialis anterior, or soleus.

Understanding the mechanics is essential: the bone adapts to stress through a process described by Wolff’s Law. However, if the stress exceeds the bone’s ability to adapt or if rest periods are insufficient, it can result in MTSS or even a stress fracture.

Is It a Stress Fracture or Shin Splints?

Distinguishing between shin splints and stress fractures can be tricky. MTSS typically manifests along the inside of the lower and middle thirds of the shin, spanning at least 5 cm. The following are common indicators of MTSS:

  • Pain often improves as you continue running (not true in severe cases).
  • Discomfort fades with rest, typically within 48–64 hours.
  • Normal blood flow to the feet with no pins and needles, tingling, or numbness.
  • No visible swelling in the area.

Stress fractures may present differently, so if you notice discrepancies or the pain persists, it’s best to consult a sports injury specialist to rule out other potential causes like compartment syndrome or vascular problems.

Why Do Shin Splints Occur?

Shin splints are an overload injury, with factors including:

  • Increased pronation: Excessive foot turning while running can strain the tibia. However, restricting pronation with certain shoes may also reduce shock absorption.
  • Body weight: A higher BMI means greater force on your tibia.
  • Ankle mobility: Excess plantar flexion can impair shock absorption.
  • Narrow step width: This increases pronation rates and tibial stress.
  • Low cadence: Running with fewer than 170 steps per minute increases ground reaction forces.
  • Hip external rotation: Greater rotation at the hip is linked to MTSS.

Tightness in muscles, once thought to be a cause, has not been supported by research. Instead, improving movement control is key.

Should You Keep Running?

In short, no, not if it hurts. Running through significant shin pain only exacerbates the problem. If your pain flares during a run (over 4/10 on a pain scale) or persists while walking, it’s time to rest.

Remember, taking a break doesn’t mean you can resume where you left off. Bone adaptation takes time, and a gradual reintroduction to running is necessary.

How Can You Alleviate Pain?

  • Stop running: Rest until pain-free while walking.
  • Ice massage: Freeze water in a plastic cup, remove the top, and gently massage the affected area for 10–15 minutes.
  • Footwear and insoles: Experiment with supportive insoles or new running shoes. Be cautious, as they may worsen symptoms in some cases.
  • Kinesiology tape: Apply K-tape under the arch and along the inside of the shin to reduce strain.

Recovering and Preventing Shin Splints

Recovery requires more than just symptom management; you need to address the root cause. Start by:

  • Offloading stress: Reduce impact exercises until symptoms subside.
  • Strengthening and mobility exercises: Target the specific risk factors contributing to your condition. Generic exercises can help, but tailored advice may be necessary.
  • Progressing gradually: Both your exercise routine and running plan should increase slowly to prevent re-injury.
